A dark granite plaque is affixed to a white stucco wall with four bolts. At the top center of the plaque, an ornate cross is etched. Below the cross, text in white lettering reads: "В этом храме с 1923 г. по 1930 г. служил священномученик Измаил Базилевский (Карагандинский). Расстрелян 17 ноября 1941 г. в Казахстане. Канонизирован 20 августа 2000 года Архиерейским собором РПЦ." The plaque is located on the Church of the Transfiguration in Skorniákovo, Russia.
